New Patient Registration
The practice will accept applications from patients wishing to join the practice unless there are fair and reasonable grounds for not doing so. The practice will not discriminate on the grounds of disability, medical condition, age, appearance, race, gender, social class or sexual orientation.
If you wish to register with the practice, please complete the new patient questionnaire and hand it to reception (or post it) with your registration form.The questionnaire will allow the practice to know more about you and your general health. Application forms and new patient questionnaires are available upon request from the reception or you can download them from the above "Registering Online" tab.
The practice will however refuse to accept patients with a history of violence, the relatives of patients with a history of violence, patients who make threats or where there is a fear of personal safety of any of the practice staff. Patients will also be refused acceptance if they have previously been removed from the practice because of an irreconcilable breakdown in the relationship or that they live outside the practice area.
Any patient being refused acceptance by the practice will be given the reasons for refusal in writing.
Register online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

New Patient Checks
We offer each new patient an appointment to have an ‘MOT’ style health check, which will be carried out by the practice nurse. The health check will consist of measuring your height, weight and blood pressure and testing a specimen of urine. You will be asked about dietary habits, smoking and alcohol intake and appropriate advice will be given. Please note that this is not a full medical examination but just a brief health check.
